Fees and Insurance

If you are contacting your insurance company, they may ask for my tax ID number and my license number, listed below:

Tax ID Number:  91-1819991

Washington Psychologist License Number:  00001434

I currently am a preferred provider with three major insurance plans in the Seattle area:  Premera Blue Cross, Regence Blue Shield, and Uniform Medical Plan.  If you intend to use one of these plans to help pay for some of your treatment you should advise me of this during your initial phone call.  In all cases where you hope to use insurance reimbursement you should contact your insurance representative first to aid you in determining benefits.

Assessments/Testing:

With a few exceptions, most insurance companies will pay for part, but not all, of the charges for a comprehensive assessment.  The table below lists the typical charges and procedure codes for a comprehensive assessment, and can be used when discussing reimbursement for your assessment with your insurance company.  The number of units (hours) and charges vary for each evaluation.  Your bill will usually include all of these tests.  Anything not covered by your insurance would be your responsibility.

Psychotherapy:

My regular fee for psychotherapy is $200 per fifty minute session.  An initial diagnostic interview is $250.  Fees for other services I provide, such as psychological testing and forensic services, including expert testimony, vary depending on the particular situation.  I reserve 20% of my practice for reduced fee work.
